What Does an Echocardiogram Do? 

The other most common diagnostic test, an echocardiogram, or ECHO, is a live imaging test that is used by doctors to examine the heart’s activity clearly. Instead of waveforms, an ECHO produces a live image of the heart and its valves through sound waves. A technician will apply gel to the patient’s chest, after which a specialised ultrasound probe (transducer) will be placed on it. The device sends high-frequency sound waves that reflect off the heart.  These sound waves eventually form detailed images of the organ. For better understanding, an ECHO can be compared to an ultrasound performed during pregnancy. 

An echocardiogram is useful for evaluating the heart’s mechanical function, size, shape, girth, and muscle motion. It is also helpful in assessing the integrity and performance of the valves. This test takes slightly longer than an ECG, typically 5-10 minutes. No pain or discomfort is felt in this case, either.

Why Your Doctor Might Order Both

Since ECG and echocardiogram both have their advantages, both tests may be ordered together to allow your doctor to have a complete picture of your heart health. Your heart can maintain a perfectly normal electrical rhythm even with a significant structural abnormality, such as a leaky valve or a weakened pumping chamber. On the other hand, you can have a heart that is structurally fine but suffers from an arrhythmia that may prove to be dangerous. 

Look at it this way. A patient may present with breathlessness and fatigue. When tested, his ECG would come back normal. After the doctor orders an ECHO, they may find a valve problem or weakened heart muscles. Had they stopped after an ECG alone, the patient’s problems may never have been diagnosed. This is why an ECG and an echocardiogram usually go hand in hand to get the complete picture. The ECG in turn may reveal the reason (a previous heart attack) for the valve leak or weak heart.

 

Which Test Is Done First?

While this depends on the situation, an ECG is typically ordered and performed first. It has many benefits. For example, it is much quicker, cheaper, and can be performed in almost any GP surgery or A&E department. Since an ECG records the heart’s electrical activity and rhythm, it helps rule out acute issues such as a heart attack or problematic arrhythmias. 

If the ECG shows abnormal findings or the patient’s symptoms are worrisome regardless of the ECG findings, the clinician may order an Echocardiogram to investigate the situation in more detail. That being said, instead of talking about “ECG vs echocardiogram”, it is better to see them as complementary and not competing.
