Hyderabad, September 23rd, 2021 : A team of Cardiologists led by Dr Karunakar Rapolu, Consultant Cardiologist, Apollo Hospitals; performed a complex interventional procedure using Impella, IVL and OCT; on an extremely critical 61 year old patient. The patient had 90% blockage due to high calcium deposit in the Left Main Vessel, which was jeopardising the blood supply to already severely weakened heart. A preliminary medical evaluation found him unsuitable for a bypass surgery, due to his past history of Coronary Artery Disease and having undergone an angioplasty three years ago. The Team had to swiftly explore for options to restore blood circulation before his condition deteriorated further and became fatal. The patient Mr Ravinder Reddy, a retired police official, presented with excruciating chest pain and symptoms of severely restricted blood flow to the heart due to calcification of the largest coronary artery, the Left Anterior Descending Artery (LAD). The LAD became narrow progressively since his previous angioplasty and manifested in the form of a massive heart attack. At the Hospital the patient was quickly wheeled into the Cath lab for an angiogram to assess the extent of heart damage. However, he developed dysfunction of the Left ventricular heart chamber, on the table, leading to sparse supply of blood to the vital organs and potential damage of those if not acted instantaneously. This episode further complicated his condition and the team had to resort to administering diuretics to rid excess fluids from the patient’s body and vasodilators to open the blocked vessels.His perilous condition ruled out any scope of performing the risk laden bypass surgery. The team narrowed down on the most apt option considering his delicate condition, of performing a CHIP Intervention (Complex High-risk Indicated Procedure) along with an Impella Heart Pump. CHIP Intervention is a highly critical procedure performed for immediate restoring of blood supply in the circulation system, while Impella replicates the work done by Left Ventricle, helps suck the oxygenated blood from the Left Ventricle and pumps it to the circulatory system. Using the infrared light guidance from OCT (Optical Coherence Tomography), the extent of occlusion was assessed and those high resolution images facilitated clearing of the blockage with precision. Intravascular lithotripsy (IVL) Balloon was used to crack the impregnable calcification of the blockage. IVL deploys acoustic pressure waves to break the calcium deposits. A stenting was done of the Left Main Coronary Artery using DK crush technique for the best outcome.During the course of the procedure the patient suffered 7-8 episodes of “Ventriculo arterial uncoupling” while inflating the balloon to clear occlusion, where the left ventricle ceased pumping the blood. Impella got into action and ensured uninterrupted supply of blood. The patient would have crashed during the procedure if Impella was not availed. The procedure took four and half hours, patient was moved out of ICU after 24 hours of the procedure. He has since recovered and was discharged on the 4th day of admission. He needs to take regular medicines and to follow healthy life style habits like regular exercises and diet management. More than 250 school children, teachers and gym instructors received training in Basic Life Support and First-Aid today at Apollo Institute of Medical Sciences and Research (AIMSR), Hyderabad. The training in Cardio-Pulmonary Resuscitation (CPR) was done by a team of Life-Support Instructors from Department of Emergency Medicine, Apollo Hyderabad, as a part of the National Assembly on Pediatric Emergency Medicine (NAPEM) conference. NAPEM 2017 is being conducted by the Society for Trauma and Emergency Pediatrics (STEP) in collaboration with the Society for Emergency Medicine India (SEMI), the Indian Academy of Pediatrics (IAP) and Apollo Hospitals. The conference is hosted with the intent of raising the awareness and standard of care for pediatric emergencies in all hospitals.Dr. Imron Subhan, Head of the Apollo Hyderabad Emergency Department, said “In an emergency, bystanders who witness an accident should provide first-aid and quickly get the victim to the hospital. Hyderabad, July 2021: A team of doctors led by renowned Cardiothoracic Surgeon and Padmasri awardee Dr. Alla Gopala Krishna Gokhale performed a rare, combined Heart and Lung transplant on 20th July 2021 to save a terminally ill twenty-five-year-old woman, Ms Labdhi Jain. The recipient was unfortunately born with congenital heart disease with a hole in her heart known as a ventricular septal defect and an abnormal connection between her great arteries (Aorto-pulmonary window). Over the years, the congenital defect led to further deterioration in her health condition with increased lung pressures and leakage of heart valves. She was surviving on oxygen support at home for the last two-and-half years. Of late, she was admitted at various hospitals several times for heart failure. The transplant team at Apollo Hospitals, Hyderabad, on evaluation found her to be in an advanced stage of heart failure coupled with increased lung pressures. The team prescribed combined Heart and Lung transplantation to save her life. She was registered under the State Government’s Jeevandan Cadaver Transplantation program and was waiting for a suitable donor. The team faced unique challenges in this case. The recipient at the time of listing for transplant weighed only 30 kgs. Getting a suitable organ which was of appropriate in size for her was complex, as her heart cavity was small. She was helplessly waiting for a suitable organ for a prolonged period of over two and half years, with the hope of finding a small-sized organ diminishing by the day.The cadaver heart and lung obtained from an eighteen-year-old girl, pronounced brain dead due to intracranial bleeding and thrombosis, aptly suited her requirement. Despite the immense sorrow of losing their child, the donor’s family displayed exemplary courage and benevolence and gave consent to harvest their braindead daughter’s organs. This humanitarian gesture on behalf of the grief-stricken loved ones brought joy back in the life of another family.The recipient recovered well after surgery; her weight has improved to 38 kgs. The immunotherapy drugs block the PD1 check point on T cells so that they cannot interact with PD1-L1 on cancer cells and cannot get killed or inactivated. With the blocked check point (PD1) the T cells kill cancer cells without a problem. In summary PD1 is sitting on our T cells and PD1-L1 is sitting on cancer cells. And there is a battle between them. KOLKATA, July 22, 2021: Beginning July 2021, a 2-month-old with almost zero eyesight was brought to Apollo Multispeciality Hospitals, Kolkata. On examination, Dr Debabrata Halder, senior consultant ophthalmologist (paediatric), found out that the child was suffering from congenital total cataract in both eyes. “In such cases, however, surgeries are carried out when children are three or four years old. And to operate on a two-month-old’s eyes is a big challenge because of the age as well as the delicate nature of the eyes”, said Dr Halder.As a consequence of having total cataract in both eyes from birth, there was a hindrance to vision development in the child. Hence Dr Debabrata Halder, who was assisted by a team of doctors, decided to operate on the child’s eyes at the age of only two months. But due to the host of other health complications like the absence of anus, fistula connecting rectum and urinary passage, and some heart defects, administering anaesthesia was a big challenge. Dr Debabrata Halder said, “The hour-long surgery has cleared both the eyes of cataract and the baby’s vision is absolutely perfect. A delayed surgery could have permanently affected the vision, which is why we had to take the chance of operating at such a young age, which is not done normally.”He also added, “For us, it was a race against time. If we would have delayed the operation, it would have affected the development of visual fixation (the capability to stare in a particular direction), which normally develops between the eighth and tenth week and we would have crossed this crucial period. Removal of bullet from liver – minimally invasive surgery/ laproscopic removal – first of its kind surgeryMr.Mahmud 50 yr male from Somalia had a bullet injury about 9 months back. Since then he was having continuous pain in the abdomen which prevented him from doing his work. He visited lot of hospitals both in his country and in India. He was advised surgical removal of the bullet. In all the hospitals he was advised conventional open surgery – wherein the abdomen is cut open and bullet along with some part of the liver are removed. He was also told that it is a high risk surgery.  He then visited Apollo hospital and after evaluating him thoroughly Doctor’s team planned a laparoscopic surgery – wherein two small holes of 0.5 cm are placed in the abdomen and with the help of specialized instruments the exact location of the bullet was identified and the bullet was removed preserving the liver and the surrounding important structures. This surgery was done with very minimal blood loss and the patient was discharged within 24hrs. post operatively patient was totally painfree and was back to his routine work.Laparoscopic removal of such a bullet injury is probably a first in the country and we at Apollo are proud to have done.With the increased expertise of laproscopic surgeons we are able to perform many such complicated and challenging surgeries safely and effectively in many such patients. The conventional or open surgery would have left a large scar about 15 – 20 cms in length and post operative recovery would have taken about a week to ten days, but with laproscopic surgery the patient does not have any scar and was discharged in less than a day.
